I haven't verified this myself, but I heard there was even shit as recently as him threatening to bring in Hazumi to deal with people who don't like Otto. Nothing forces you to pay attention like a "If you ignore or go against my my prestige level Shinigami gon' attack you." statement.

It's not a question of who is and who isn't at this point. Rania hasn't gone anywhere IC. Maybe Mika will be back, maybe she won't, but you'll have to work your way up to a point where the Commander sees fit to promote you to command regardless.
For now, the division still has a captain, but it can safely be assumed she's just running things at the status quo pending the player's return.
